Prescription & Insurance Forms 1. Fax these forms to the Pleurx Specialists at Edgepark Medical Supplies: (330) 963-8576. We recommend that these forms be completed and submitted three to five days prior to catheter placement to ensure the availability of drainage supplies. 2.
